An Introduction to Trevor Stanhope's Care

This report outlines the support provided to Trevor and highlights the positive outcomes achieved through person-centred care. Trevor is registered blind and one of the key outcomes identified within his needs assessment is to ensure that he is able to regularly access the community and maintain an active lifestyle outside of his home environment. 

Maintaining community access is important for Trevor’s independence, wellbeing and social engagement. Due to his visual impairment, Trevor requires guidance and reassurance when navigating outdoor environments such as pavements, road crossings and public transport areas. 

Support staff work closely with Trevor to ensure he can continue accessing the community safely while maintaining as much independence as possible. Through structured routines, consistent support and encouragement, Trevor is able to participate in activities that are meaningful to him while remaining confident and safe.
